We would like to extend a warm welcome to our practice. We are committed to doing everything possible to provide you with high quality, state-of-the-art periodontal and implant care and also to make your visit to our office a pleasant experience.

Referrals

You do not require a referral from your dentist to be seen at our office. We always welcome referrals from friends and family and consider it the greatest compliment when our services are recommended. If you were referred to another periodontist but selected our office instead because we are in-network with your insurance, simply advise your dentist to forward your records and referral to us.

What to Bring on Your First Visit

Please assist us at the time of your initial visit to the office by providing the following information:

  • Your referral slip and x-rays from your referring dentist.
  • A list of medications you are currently taking (you can download our patient registration and medical history forms below)
  • If you have dental insurance, please bring any forms or insurance cards with you to the appointment.

Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring premedication, severe diabetes, or hypertension.)

If your referring dentist has taken x-rays, you may request that they be forwarded to us.

Antibiotic Pre-medication

Patients with certain heart conditions or artificial joints may be asked to pre-medicate with antibiotics prior to treatment. We do this with your safety in mind. If you have a condition that might require pre-medication please check with your physician for specific instructions. In absence of specific instructions, we will follow the American Heart Association recommendations (www.americanheart.org/presenter.jhtml) to give you the most up-to-date care. 

Post Treatment Instructions

After treatment, home care and medication instructions will be thoroughly explained. For your convenience, you will also be given printed instructions. We are also very available for questions after treatment.
